  4. Automated Reminders: Automatically notify the HR team via email when feedback is submitted, prompting them to review the content.

The questions on the form match the columns in your database.

  1. Go to the Employee Feedback Table.
  2. Add a new column (e.g., "Single Select" for a 1-5 rating, or "Long Text" for comments).
  3. Open the Submit Feedback Form view and drag the new field onto the form canvas.

2. How do I set who receives the email notifications?

  1. Go to the Automation tab.
  2. Select the "Automated Reminders" workflow.
  3. Click the "Send Email" action node and enter the HR team's email address in the recipient field.

3. Can I make the survey anonymous?

Yes. When configuring the Submit Feedback Form, simply ensure you do not add the "Name" or "Employee Link" field to the form. This allows employees to submit data without it being tied to their profile in the Employee Directory.
